100 club application form

To become a member of the "100 Club" and have the chance of winning £160, £80 or £40 each month, as well as supporting the development of the Club, all you need to do is:

1. Complete the standing order form below and send it to your bank.
2. Inform David Jack , who will then allocate you a number.

When the first payment goes through to the 100 Club's Account your number will be included in that month's draw and all subsequent draws while payments continue. The monthly draw is carried out at the Committee meeting, which is usually the last Thursday in the month.
